server/gdb: Use LOG_TARGET_xxx() to show target name

The output "gdb port disabled" is confusing without reference to the
target. Use LOG_TARGET_INFO() to output the target name.

While at it, use LOG_TARGET_xxx() for all log statements where the
target name is already used.
